Re:        Workers’ Memorial Day            

As you know, every Workers’ Memorial Day (WMD) on April 28, CWA commemorates our members who were killed on the job over the past year. In preparation for the upcoming Workers’ Memorial Day, we are gathering information of CWA members’ work-related deaths that have occurred since last year’s WMD (April 29, 2025 – present). We need your help to make sure we do not miss anyone.

Please submit the following information by Thursday, April 16, 2026, to ensure that all CWA members who lost their lives are remembered and honored:

  • First and last name of deceased member
  • Age
  • CWA Local Union #
  • CWA Local Union position (if any)
  • Name of employer
  • Member’s job title or position
  • How many years the deceased member was employed at that job
  • Date of death
  • Date of the workplace incident that resulted in the fatality (if different from date of death)
  • Location of workplace incident/accident and address (if known)
  • Description of the incident
  • Do you believe the fatality was work-related?
  • Please add any additional information you would like about the deceased member, such as information about their family, number of children, etc.
  • Name and e-mail address of the person submitting this information
